These Terms of Service (“Terms”) govern your use of the website, patient portal, and telehealth services operated by Radhaimilda Cuevas Matos, FNP under the brand RC Family, Virtual Care & Wellness(“we,” “us,” or “our practice”), located at 100 N Orange Avenue, Suite 200, Orlando, FL 32801. By accessing our website or using our services you agree to these Terms. If you do not agree, do not use our services.
1. Services provided
We provide telehealth consultations, primary care, medical weight-loss management, IV therapy, and aesthetic services as described on our website. Our services are available to patients located in the state of Florida and wherever otherwise permitted by applicable law. We do not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
2. Telehealth consent
Telehealth services involve the delivery of health care using electronic communications. By scheduling a telehealth appointment you consent to receive care through this modality and acknowledge that: (a) you understand the limitations of telehealth compared to in-person care; (b) you have the right to refuse or withdraw from telehealth services at any time; and (c) your provider may determine that telehealth is not appropriate for your condition and refer you to in-person care.
3. Not a substitute for emergency care
Our services are not a substitute for emergency medical care. Do not use this website or patient portal to communicate urgent or life-threatening medical concerns. Contact 911 or go to an emergency room immediately for emergencies.
4. Appointments and cancellation
Appointments are confirmed upon receipt of a deposit payment. You may cancel or reschedule without penalty with at least 24 hours' notice. Cancellations with fewer than 24 hours' notice or no-shows may result in forfeiture of the deposit. We reserve the right to reschedule appointments due to provider availability or technical issues; we will notify you as soon as possible if this occurs.
5. Payment and billing
A deposit is required at the time of booking and applied toward your visit fee. Remaining balances are due at the time of service. We accept major credit cards processed securely through Stripe. We do not store full payment card numbers. Applicable taxes and fees will be disclosed before payment is completed. Insurance billing varies by plan; you are responsible for confirming coverage before your visit.
6. Privacy and HIPAA
Our collection and use of your health information is governed by our Notice of Privacy Practices and Privacy Policy. By using our services you acknowledge receipt of our HIPAA Notice of Privacy Practices.
7. Patient portal and account security
You are responsible for maintaining the confidentiality of your portal login credentials. You agree to notify us immediately of any unauthorized use of your account. We are not liable for losses resulting from unauthorized access caused by your failure to safeguard your credentials. Multi-factor authentication is required for clinical staff accounts and recommended for all patients.
8. Intellectual property
All content on this website — including text, graphics, logos, and software — is owned by or licensed to RC Family Care and protected by applicable copyright and trademark law. You may not reproduce, distribute, or create derivative works without our prior written consent.
9. Limitation of liability
To the fullest extent permitted by Florida law, RC Family Care is not liable for indirect, incidental, or consequential damages arising from your use of our website or services. Our total liability for any claim will not exceed the amount you paid us in the three months preceding the claim.
10. Governing law and disputes
These Terms are governed by the laws of the State of Florida. Any dispute arising under these Terms will be resolved in the state or federal courts located in Orange County, Florida. You waive any objection to jurisdiction or venue in those courts.
11. Changes to these Terms
We may update these Terms from time to time. The current version will always be posted on this page with the effective date. Continued use of our services after an update constitutes acceptance of the revised Terms.
